    In partnership with Trilogy Care, we provide personalised Care Coordination for people choosing to self-manage their Home Care Package. We help you organise your services, understand your options and get the most from your funding, while keeping choice and control at the centre of your care.

A Home Care Package helps older Australians continue living safely and independently at home. In partnership with Trilogy Care, Sammi.K Support Services provides personalised Care Coordination for self-managed Home Care Packages, helping you organise supports, understand your options and make the most of your funding.

CODA Auslan Interpreting

As a CODA (Child of Deaf Adults), the Deaf community and Auslan have always been a big part of my life and are incredibly close to my heart. With lifelong experience, understanding and connection to the Deaf community, I provide Auslan interpreting with warmth, respect and genuine understanding.

Interpreting is available face-to-face or remotely via Video Remote Interpreting (VRI), including Zoom, IMO and FaceTime, making communication flexible and accessible wherever you are.

NDIS-funded Support Coordination

Support Coordination is about helping you understand your NDIS plan, make informed choices and get the most from your funding.

We can support you with:

  • Understanding your plan and working towards your individual goals

  • Connecting with the right disability supports, allied health professionals and community services

  • Coordinating your supports and helping everything work together

  • Building your confidence to navigate the NDIS and make choices that are right for you

To access this service, your NDIS plan will need to include funding for Support Coordination.
